A cremation can only be carried out with the necessary authorization and permits. California law states that only the person who has the responsibility for disposition arrangements can give that authorization. This person is often known as the authorizing agent.

Below are the various legal forms that the authorizing agent must complete for West Coast Cremation and Burial Services to perform any cremation. Some forms relate to immediate need; others relate to pre-planning a cremation.

General instructions for completing the forms

As soon as you have placed an order for a cremation with us, we will send you an email confirming which forms you need to complete:

  • Go to the relevant form below
  • Click on Download Form to print.
  • Enter the essential information and sign.
  • Scan the signed documents and email them to us at info@wccremation.com, or mail them to West Coast Cremation and Burial Services at PO Box 1077, Hemet, CA 92546 using first class or registered mail.
